6/23/2023 EnchantaVaughn Approved (AM) upon arrival, client was observed sitting on the carpet listening and attending to his reading time. Client was observed keeping his hands to himself and being respectful to his peers. Client was on task and avoiding conflict due to following simple instructions and using proper voice volume. Client was monitored and supported for displaying age appropriate behaviors. Client was seen in interacting well with his peers during group activity. Client appeared to be on track with his goals and objectives. Client communicated positive feelings while participating in each of his assignments. (PM) Teacher reported she has to continue to remind client about making the right choices and expressed she has not seen any new changes in his behavior since returning back from suspension. Client was taught to raise his hand and ask for permission before leaving his seat. Client was observed following directions and correcting his mistakes. Client transitioned to lunch in an overly energetic mood, as evidenced by him spinning in circles in the line and constantly making noises. Client was directed to face forward and walk quietly. Client communicated that he wasnât doing anything wrong and wanted to prove he could act civilized without getting into trouble and reframed from hyperactive behaviors. (EOD) Client was seen being polite and courteous to peers and staff. Client expressed good communication by saying please and thank you. Client was seen sitting at his table and engaging in conversation. Client was excited to attend his psycho social, as evidenced by his smile and happy tone. Client displayed appropriate attitude in session and returned to class in a good mood. Client prepared for dismissal and announcements. All objectives were met due to eliminating aggressive and impulsive behaviors. (AM) QMHP had one on one with client. QMHP reviewed yesterday's activities. QMHP provided support and encouragement to ensure goals and objectives are met. QMHP monitored client individualized behaviors in collaboration with his teacher and social environment. QMHP encouraged client to make good choices and to avoid negative thoughts about himself. QMHP processed client positive behaviors. (PM) QMHP assisted client in developing more social skills. QMHP inquired about possible challenges client can experience with applying this skill. QMHP provided positive feedback. QMHP reflected on client goals and objectives. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce episodes. QMHP assisted client in normalizing the changes that are occurring in school. (EOD) QMHP explored the clientâs capability to cope with his stressors and to get back on track. QMHP modeled the correct behaviors and conduct during quiet time. QMHP assisted client with finding and utilizing necessary resources. QMHP verbalized the awareness that all people are unique, learn differently, and have various strengths and weaknesses. (AM) Client reflected on weekend activities. Client remembered what we discussed yesterday and plans to apply those strategies today. Client was acceptive to support and encouragement. Client expressed positive thoughts about himself. Client was seen making good choices. Client communicated by nodding and exhibiting good eye contact. (PM) Client was displaying good social skills by interacting well with peers. Client is still learning to build relationships and contribute to conversation. Objectives were being met at this time, dispite stressful environment. Client walked away from others who were doing wrong. Client acknowledged interventions and verbalized clear understanding. Client expressed positive and negative feelings about changes occurring at school. (EOD) Client engaged in reflective thinking. Client communicated the do's and don'ts. Client was acceptive of feedback. Client is back on track with goals and objectives. Client was able to comply with authority rules and regulations during quiet time. Client utilizes help and support. Client communicates and displays unique traits and says his strength is his confidence (how he feels about himself) and his weakness is his self-esteem (how others feels about him) and is working to improve everyday.
